I, {first_name} {name}, hereby acknowledge that I am participating in Brazilian Jiu-Jitsu classes, training, and/or other activities (the "Activities") at Corvus Jiu Jitsu (the "Gym") at my own risk. I am aware that BJJ training involves physical contact and strenuous physical activity that may result in serious injury or even death. I understand that the risks of participating in BJJ training include, but are not limited to, broken bones, concussions, joint dislocations, and other serious injuries.

In consideration of the opportunity to participate in BJJ training at Corvus Jiu Jitsu, I voluntarily assume all risks and hazards associated with my participation in such training.

I hereby waive, release, and forever discharge Corvus Jiu Jitsu, its instructors, employees, and agents, from any and all claims, actions, suits, damages, or liabilities arising from or related to my participation in BJJ training at Corvus Jiu Jitsu, including but not limited to claims for personal injury, property damage, or wrongful death.

I understand that I am responsible for my own safety during BJJ training and agree to comply with all rules and policies established by Corvus Jiu Jitsu regarding such training. I acknowledge that I have received and read the rules and policies and understand that failure to comply with them may result in my expulsion from the gym.

I hereby certify that I am in good health and have no physical condition that would prevent me from participating in BJJ training. If I have any pre-existing medical condition that may affect my ability to participate in BJJ training, I have disclosed it to Corvus Jiu Jitsu in writing.

I agree to indemnify and hold harmless Corvus Jiu Jitsu, its instructors, employees, and agents, from any and all claims, damages, or losses that may result from my participation in BJJ training at Corvus Jiu Jitsu.

I have carefully read this waiver and fully understand its contents. I am signing this waiver freely and voluntarily and without any inducement, assurance, or guarantee being made to me.

I understand that the Gym has the right to terminate my participation in the Activities at any time, for any reason.
